National Diabetes Support Team ‘Showcase’ Conference

Please find attached an Application Form and Outline Programme for the National Diabetes Support Team ‘Showcase’ Conferences due to be held in Leeds 13 November and London on the 13 December.

There is no attendance charge for the conference which will cover the following topics:
